ACCEPTED FORMS OF PAYMENT- We require payment in full at the time you book your appointment. We accept credit cards, debit cards and health savings account (HSA) cards. We do not accept personal checks. Please note if you are using a HSA card or paying with a flexible spending account (FSA) you need to read the instructions located below the appointment scheduler button. We also accept cash for items or services that are provided when patients are in the office. SCHEDULING AN APPOINTMENT- Patients may reserve an appointment directly via our website at any time (24 hrs/day, 7 days/wk) without assistance from our office. Our automated scheduling software offers greater convenience, efficiency and privacy to our patients. Patients may also reschedule their appointments directly via the same software as long as it is more than 24 hours before their appointment time. Payment for the visit will be collected online at the time the appointment is reserved. If you have a health spending account (HSA) card or a flexible spending account (FSA) please read the information listed below our scheduler button before reserving your appointment. Patients should contact our office manager via phone, text or email if they need to cancel their appointment. A refund will processed for cancellations. Patients should allow 10 business days for processing of refunds via their bank or financial institution. Please contact our office manager or receptionist if you have questions or need assistance.
RESCHEDULING AN APPOINTMENT- Patients may reschedule their appointments without assistance as long as it is more than 24 hours before the time they are scheduled to be seen. To reschedule the appointment go to the appointment confirmation notice you received via text or email, there you will find instructions for rescheduling. If you need to reschedule less than 24 hours before your appointment contact our office for assistance. A $50 no show or late cancellation fee may be applied if you are unable to keep the appointment you originally scheduled if less than 24 hours notice is provided.
CANCELING AN APPOINTMENT- Patients need to contact our office if they need to cancel their appointment. Refunds will be provided but a $50 no show or late cancellation fee may be assessed for cancellations with fewer than 24 hours advance notice. Refunds may take 10-15 business days to reflect on your credit card or bank statement. OFFICE VISITS-All providers at Nova Wellness Center are considered out of network with all insurance plans. Both Dr. Sachdev and Lauren Sullivan are not contracted with ANY private insurance companies, Medicare, Medicaid, CHIPS, or Tricare. They provide service to patients on a private pay basis, which means that patients pay out of pocket for any services provided at Nova Wellness Center. Payment for services rendered is collected in full on the day that the appointment is scheduled or the day the service is provided.
NO SHOW FEE- We assess a $50 fee for missed appointments or cancellations with fewer than 24 hours notice.
LATE ARRIVALS - You may be asked to reschedule your appointment if you arrive more than 15 minutes late. We will do our best to accommodate you if you miss your appointment but cannot guarantee that you will be seen on the same day.
TEST RESULTS- Dr. Sachdev:She sends test results to her patients via email within 10-14 days of patients submitting their specimen. Patients should not assume that their results were normal if they did not receive their results, they should contact the office to notify staff that they have not yet received their results. She may require patients to schedule a follow up visit to discuss results that require more time to address than an email can provide. Lauren Sullivan, PA-C: Reviews test results with her patients during an office visit, an appointment is required. Patients who have access to our patient portal may be able to view their test results as soon as they are available but will need an in office or telemedicine appointment to discuss them with Lauren Sullivan. Patients should not assume that their results were normal if they did not receive their results, they should contact the office to notify staff if their results are not available within 14 days of submitting their specimen.
PRIOR AUTHORIZATION FOR MEDICATIONS-Patients should contact our office directly if their pharmacist notifies them that their insurance company requires "prior authorization" (PA) of a particular medication. We do not recommend waiting for your pharmacy to contact our office. Our office manager prefers a text message to our clinic phone 281-542-7800 or an email to SachdevMedAssistant@gmail.com as the most efficient means of communication. PA processing typically takes 1-10 business days depending on your insurance provider. There is a $15 fee assessed by our office to process a PA request. There is no guarantee that your insurance company will approve the PA request for medication. The fee is assessed irregardless of your insurance company's decision to allow or deny coverage of your medication. The fee is assessed to cover our cost for labor to process the required paperwork for you. MEDICAL RECORDS FEE- A $5fee will be charged to provide patients with copies of their medical records. Please allow 3-15 business days for our staff to process your request. If patients need their records immediately they should contact our office manager directly via voicemail, text message or email to notify her.
FORM COMPLETION FEE- Paperwork that requires completion by Dr. Sachdev, Lauren Sullivan, PA-C, or her staff, outside of an office visit may incur a fee. Examples of paperwork that are subject to this fee include but are not limited to: FMLA forms, return to work forms, health or life insurance application forms, orders for a school nurse to administer medication to a student, and impairment assessment for special education services. Forms other than the ones mentioned may also be subject to a fee, including letters written by the physician or healthcare provider. The fee will range from $10-30 depending on how much time it takes Dr. Sachdev or Lauren Sullivan to complete the paperwork or letter.The fee is assessed per Dr. Sachdev's or Lauren Sullivan's discretion and is assigned on a case by case basis. Our staff will notify you if a fee is likely to be required for the paperwork that you are requesting.
VACCINATIONS- Due to inadequate reimbursement and heavy administrative burden we no longer provide vaccinations. Patients will be referred to their local pharmacy or urgent care facility for vaccinations.
**Patients who are on BioTe therapy and need to contact Dr. Sachdev regarding an issue related to BioTe should contact Dr. Sachdev directly by texting her mobile phone. Her mobile number is listed at the bottom of the instruction page provided during the first procedure appointment.
Providers at Nova Wellness Center do not take after hours call and do not care for patients in the hospital or emergency room. If you believe you need admission to a hospital you should go to your local emergency room for evaluation. An emergency room physician will evaluate you to determine whether you need to be admitted. If you do not have another primary care physician who has admitting privileges at the hospital then a hospitalist physician will be assigned to you.
